Table of Contents
Submitted by a shitpot full of people, the FDA has gotten this wonderful idea to allow people (read: idiots) to buy Rx prescriptions OTC using a kiosk rather than a Doctor to issue the Rx.
The jist of the article (for those too lazy to read it), is that you would go to a kiosk, and via a handy flow chart be able to obtain an Rx for antibiotics (!!) hypertension medication, cholestrol medicine, etc without seeing a doctor. Your helpful pharmacist would help you from then on out. Yeah, as if that’s going to magically make you become more compliant. I’ll get right on that after my job as the vicodin-police, insurance agent, and your personal fucking nanny are done.
Lets take a journal into sarcastic TAP-land:
I think this is a completely fucking great idea! We all know that Rx medications are completely safe, require no professional monitoring, and can cause no harm to the patient. I mean who in the fuck cares if your potassium goes through the roof when you’re on an ACE-I, or if you get rhabo while on that statin. This whole Rx concept is obviously a cartel plot by the medical industry to milk you out of copay money to line the pockets of BMW driving doctors as they light their cigars with your crisp $100 bills gotten from your copays. Its not like those doctors know any more than the crackhead off the street…..
Shit, this whole idea of people just making shit up at kiosks to get a truckload of antibiotics is such a good idea! We all know that antibiotic resistance is just a big fucking lie made up by the pharmaceutical industry so they can push their ‘newer’ expensive antibiotics to leech the consumer dry. In fact, all of medicine is a big fucking lie meant to milk you out of your hard earned cash. See this MD/PharmD degree? Thats just made up letters that we got from a mail-order school.
Travelling back to real-life common-sense land:
This whole clusterfuck was meant to save the patient money they would have spent seeing a doctor. Because thats SO MUCH FUCKING LESS than say a hospital admit because your potassium shot up to 12, your kidneys stopped working because you have renal issues and had to get that ACE-I, or because MRSA is eating your face off. Not to mention the tons of other issues that come with taking A CONTROLLED FUCKING POISON that externally influences certain enzymes and receptors in your body thereby circumventing your body’s own natural regulation pathways. Oh shit, you didn’t know that most (if not all) drugs did that? Actually I’m sure you probably thought medications were made from unicorn farts that magically made you better.
Here is a fucking great idea, why don’t you use your doctor to DIAGNOSE whats wrong with you, and have the pharmacist PICK OUT THE RIGHT MEDICATION. I mean we all know that 90% of our time is just faxing the doctor for a drug change because its the insurance company, not your doctor who decides what drug you get. Tell us whats wrong with you, any lab values that we might find useful, and let US pick out the most effective and affordable drug. Pharmacists are immune to the drug-rep masturbation, we (for the most part) know our shit, and our ass is on the line with you by default if the shit goes south. The hard cold fact is that MD’s are good at figuring out whats wrong with you, and WE are good at what drugs to use. Same coin? Different side? Sound familiar? Obviously not to the FDA.
If the APhA is all about this, its just more evidence that they are focused at planting new pharmacy trees while the retail pharmacist forest behind them is burning to the ground. Why are they always ignoring what needs to be fixed in lieu of added work for no added money for us?
Oh, and if you are thinking “Well it works in Mexico”, take a second and think about what happens if you take something and die in Mexico. Do you get to sue someone? Do you get compensation from the manufacturers? Nope, you get a dirt-nap and a “Oh thats too bad”.
But really, what the fuck do I know. I just count by 5’s all day while watching Dr Oz and drinking coffee.

What a wonderful rage. The system is totally corrupted and broken. I feel your frustration. Why did we bust our asses to learn the science? Why did we bust our asses to stay on the right side of the law? Because it was required to obtain our degrees and positions. And for what? So we could serve uneducated scum baggs and serve under very intelligent (yet totally corrupted) self-serving business-first politicians, legal eagles, insurance companies and enforced by an equally corrupted police state…..When the shit finally hits the fan all you’re gonna hear is “I was just following orders” and the “chiefs” who gave the orders will never be divulged let alone do jail time. That is the lesson to be learned in the “NEW U.S.A.” Hell, the old and rediculous systems (Government, Religion, etc) are yet being supported by a populus of mentally retarded people….who can’t face the truth.

What really bothers me about this story is the motivation behind the FDA’s idea. They are under political pressure to make changes that the government hopes will rein in Medicare costs. This decision isn’t being considered based on the merits of having prescription drugs sold directly to patients or the safety and/or monitoring issues involved with giving out prescription directly to consumers. IF they really took a look at what drugs could be considered safe to be sold without a doctor’s prescription and then created a safe bridge to the consumer in the form of a pharmacist controlled behind the counter drug class, I could get on board with this ease of access idea. As it stands, the government is deperate to save healthcare dollars even if that means pressuring the FDA into making foolish decisions. This consideration by the FDA is more a testament to the fiscal health of our nation more than a public health or policy proposal.

There are two sides to this problem.
First, if patients are allowed to purchase prescription medications over the counter, pharmacists all over the country are going to have another job added to their laundry list of things to complete while simultaneously being expected by their employers to fill prescriptions at the rate of no less than two per minute. For you all, this is going to suck. And … if the patient does develop a serious side effect from the medication, then they will blame the pharmacist because no physician was involved in prescribing the medication.
On the other hand, I don’t see a problem with cutting out the barriers to patients getting some medications. Using your example, people in Mexico who can get many drugs over the counter don’t have a higher rate of kidney disease than people in the US. http://www.nature.com/ki/journal/v68/n97s/full/4496424a.html.
Let people get their medications (except antibiotics and controlled substances) over the counter. When enough people start dying because they took the wrong dose of medications or mixed the wrong medicines together, then others will begin to see the value in the services that pharmacists and doctors provide.

Your proposed system is great in theory. I wish pharmacists could use their skills like that, but in a society when people can’t be bothered to bring a wallet sized insurance card with them to the pharmacy, do you really trust them to bring lab results and charts? Or do you trust them to only bring you the lab results so that they can’t obtain multiple rxs for antibiotics? Let’s be honest, people are just too dumb and manipulative to have pharmacists be the only check and balance in the system. Plus when are you going to have time to interpret these values, verify, counsel, give flu shots, do various corporate bullshit, and tell people when the bread is?
I wish pharmacists had the respect to get this system implemented in the community because it makes the most sense and uses people’s skill to the greatest potential.

Bottom line is that they want patients to pay for medications out of pocket. I’m sure insurance companies are extremely involved in the process. However most of what they are talking about is out there already. Allergy – most antihistamines are already OTC ( most insurances don’t cover OTC ), headache – Advil ( are they going to put ibu 600 OTC? Big fucking deal!). I doubt fioricet will go OTC. Asthma- why was primatene pump discontinued? Cholesterol – red rice yeast , already populized that it has statin. Blood pressure medication – bad idea! Go pick out inie minie minie mo. The computer is gonna decide if pt needs a beta blocker or ace inhib? What else? Antibiotic? Maybe not a bad idea, but how can you trust a person to figure out where the infection is? Or computer is gonna perform a strep culture test? We can’t trust a patient to remember his sons date of birth or tell us what kind of cough he/ she has and we gonna let them answer the questions correctly? But we all know better. It’s gonna come down to pharmacists doing more work for no extra money. We gonna have to read the questions to them, answer for them and then get yelled at afterwords why they have to pay if they have insurance.

Actually, I think the way to reduce healthcare costs would be to make Lortab OTC in this type of environment. Think of all the Medicaid patients who hit up local ERs for Lortab every few days… Make a maximum quantity of 10 per 24 hours or something. It’s relatively safe as long as the patient doesn’t consume too much acetaminophen, and few are really allergic to hydrocodone. I don’t think going to an annual exam to get a lisinopril script is driving up healthcare costs.

When I’m burning up, my teeth are killing me and I have yellow-green snot spuing from my lungs, I sure don’t need to waste $130 on a docyors office visit to know I have a BACTERIAL infection. Your rant is a counter conspiricy, covering up the fact that pharmacists are the ones who really wanna be Rxing drugs to patients-just like they do in Washington State. Come on, I mean really; do you really want me coming into your pharmacy and show you a snot rag full of infectious puss just to get a Z-Pac? That means getting drugs only from the clinic itself. These mega malls of medical magicry are in cahoots with the insurance and drug companies to cut you, the pharmacist middleman, out of the picture. Go back to school and get that radiographer or ultra sound technician degree. You’ll be happier to be part of the medical profession and at least have jobs. I mean, please tell me this won’t happen!!! I do like your idea of doctors diagnose/pharmacists choose the right medication as I’ve come very close to this happening on a number of occasions–at the very least, my incredibly knowledgeable pharmacists have called my doctors questioning certain interactions and dosages and in each and every time, the doctors have deferred to the pharmacists. Could be because it’s a small-town situation where everybody knows everybody, but to me a pharmacist should be held in the highest regard. Just saying…

What the FDA seems to be unaware of, is that:
A – The damned Prescriptioned Medication isn’t like paracetamol that You can take as many as You like without having any adverse reactions (OHHH! Wait a second! liver necrosis… right…) Aspirins out of the picture too… I mean… EVERY DRUG IS DANGEROUS.
B – I’ll bet that every hypochondriac in the states will get 50 drugs for the 50 illnesses he “has” diagnosed for himself… Interactions might be a problem too.
C – With bad use of medication comes more wasted medication, more resistent bacteria, more negative drug effects and more cost to the government in keeping these problems at bay. (HMOs won’t lift a finger, i’ll bet).
In all, I think it’s going to be a disaster in Health management. I suppose there are doctors and pharmacists in the FDA… but they might have some different ethics than the ones we are supposed to practice.

If you’ve wasted your Salie Mae student loan and are miserable, any of you, too bad so sad go fuck yourself. People (me) trust “you”. With our lives. All the while your mumbling under your breath usually about someone’s hair-style makes their face look like a beachball, and I don’t have to point out at what else oO(the obvious, the self-evident, the politics, accounting, the faxes, the forms, the addicted, on & on etc.), while all the while (as are school teachers for example, you are underpaid, understaffed, underappreciated, but I, the one with the prescriptions that add to my years, quantity & quality trust you, as the position that you’re in demands that) “you” as it turns out are no different, or better than the waiter/ress with the side of spit (saliva, for you to good to read spit), or the co-worker that puts the cream in ones coffee (that’s jizm as I’m certain none of you, male anyway, have any sexual experience whatsoever or else you wouldn’t be so angry), or the mailman in the bell tower.
I have one word for “you” all, angry pharmacists.. suicide. Try it! The world will be a better place. And don’t procrastinate.
I fucking hate all of you whom violate my trust as if your wearing a name tag, oh wait.. you are, and leaning out a window, oh wait.. you are, with a bag of fries in your hand and THOSE possibilities and probabilities in life. Well, sir… regret to inform you that you don’t understand the pressure we’re in nor what we had to study to be at the counter of a pharmacy. We have to make sure you guys (pharmacy costumers and some doctors patients) don’t kill yourselves with your own medication, cause it’s our job. Maybe we might be a little unhappy if we tell almost everyday of our lifes the same info for every drug thats dispensed.
We are unhappy because people like YOU don’t appreciate our jobs of treating you with respect although you guys don’t give a shit what we say or know unless you get a problem with your medication and it’s all our fault for giving it to you… well fuck that… we tell everyone how to take their medication, we give information on how to treat yourself good and even can manage to see if your condition has improved or not to treat you better next time and yous guys still respect us… NO!
If the costumers want a medication I’d be happy to give it to them if they give me a proper justification, if not: trust us, we’re the specialists! With this type of deal from the FDA, You can diagnose yourself, take the medication the “machine?” will tell you or other one of your chosing… just remember: pharmacists won’t have a say if things go sour… and they will. Let’s do our job, be grateful we don’t kill everyone for not knowing our shit, and hope that someday when shit hits the fan we’d have something to help you still.

On further review, no. I quit. If anyone reads these, cryptic or riddle as they may be (because jesus that one reply had absolutely nothing to do with anything I attemted to say, attempting to make a point), if anyone figures it out, awesome.
Bottom line is most of you should be ashamed of yourselves and I can not comprehend in the depth and width of my mind how you can look in a mirror or sleep at night (some of you, day). I have this disease, it’s called a conscience, and this no matter how correct I am and passionate is uncomfortable to me.

Well, there is a lot of more should be on the list to be OTC, such as vicodin, Norco, percocet, Oxycotin, Adderall, Methadone, any not mentioned C-II. I am sure it will reduce 99% of visits to pain management clinic and so-call urgent care

Being a druggist is soon to be an obsolete profession. YOU ARE THE MIDDLEMAN! The trend is to cut the middleman out of the picture. Drug companies pre-package meds, machines to automaticalloy count and fill drugs, drug vending machines in the ER, mail order Rx, yea, I see the writing is on the wall. Pharmacists converting to become CNAs or hosspital valets is the trend o’ the future.

Out of professional curiosity, why is it that so many RPh’s deem themselves “Vicodin Police”? I am a healthcare professional and quite familiar with the rampant Rx abuse and fraud, but I am also a patient who has been screwed over by RPh’s. I’ve noticed this is much more common with those holding a BScPharm than compared to a PharmD. For instance, I have a chronic condition which requires opioid therapy. Multiple times following submission of a valid and timely hydrocodone script for dispensing at a pharmacy, BScPharm RPh’s have refused to dispense the C-III. On one occasion I requested a refill of Norco 10/325 #30, but was only able to get half (15) of the script (Sig Norco 10/325 t PO Q4-6 PRN). Several days later when I attempted to get the rest of the partial fill, the RPh stated that the 15 I had received 2 days prior was to last me 15 days, and I would need to wait a 15 day period from the date of the partial fill. Now why would 15 Norco be intended to last 15 days, with specific Sig Q4-6? The way my math comes out, the 15 Norco should last between approximately 1.5-3 days if taken Q4-6. As a professional I am quite opposed to Rx abuse, but that doesn’t mean that RPh’s should act like they are Nazi Gestapo towards someone like myself with valid scripts, valid pathology warranting them, and a colleague in healthcare to boot…

Statins are pregnancy category X, so NO to that one. Omeprazole is already OTC. OCPs – no, but maybe a hybrid class? If a patient is not doing well on one, RPh’s could switch pt to another as long as pt goes in for yearly pap smear, rx is good for one year, no matter which one? Some patients do well on monophasics, other better on triphasics.
While we are at it: DAW 1 on any prenatal vitamin is the proof of the drug rep manipulation you wrote about…..especially if my tax dollars are wasted on it.

If your condition is chronic, why is the Dr. only prescribing a relatively short (5 day) supply of the medication with refills? There really is only one reason why your treating Dr. would do that. He/she is trying to limit your access to the meds because they don’t trust you with a month supply.
Being a “healthcare professional” and knowing some terminology does not exempt one from being a crackhead.
P.S. How do you know which RPh’s have a B.S. and which have a Pharm D?

Hey genius. Read your rx the next time you get one. When it says “take 1-2 tabs every 4 to 6 hours FOR 30 DAYS”, that means I cannot dispense more than whatever the quantity is less than every 30 days, whether it’s for 20 pills or 200 pills.

Obviously you must be a new grad or work for a pharmacy organization since you claim to believe you ‘work for a company that cares and things are going to change’. So basically the verbal contract my husband was given more than 26 years ago when he decided to defend your rights and TAPs to freedom of speach means nothing to the federal government who has turned a blind eye to the veterns but makes sure that the federal employee’s insurance program (which also insures your congressional and senate representatives) allows our ‘public servants’ to see any doctor they want (including specialists) and get any med they ‘need’ without a prior authorization (hope you know what that means). I got into pharmacy because my daughter became part of a FDA trial to approve a orphan drug that 50 (yes fifty) people in the USA are on.
Sorry for not being supportive or understanding, but everything he says is true. Hospitals, and their organization, think that pharmDs without a residency are stupid and that those who work in retail must be idiots. That is far from the case. Retail pharmacists are the first line for the tight wads who don’t want to pay a copay to their MD but want us to diagnose whatever nasty skin disease (so far I have been flashed twice this year so that I could see what I don’t want to see) they have or suggest an OTC for the aweful eye infection they obviously have. We are also the ones who tell MD’s ‘hey, that antibiotic dose for that 8 month old is too high’. So, sweetie, if you don’t like the blog, then don’t read it. Go back to your cubical where you are planning to change the world and let us vent over the idiots who let their medicaid lapse, want us to diagnose their condition and treat it with an otc, and protect our patient’s from MD’s who should not have the right to prescribe if they can’t spell the name of the drug.

Hey there, I’m in a bit of a picle and need to ask you something please. My doctor has given my Opana IR pill way to early and I can’t get any pharmacy’s in North Carolina to fill the script. What I was wondering is I could get them done in South Carolina? Have you ever heard of this being done? I feel like a total “CRACKHEAD” but as much pain as I stay in I need these pills really really badly, so anyhelp you go do would be wonderful. Just yesterday, a patient came to my pharmacy complaining of prolonged weight loss. She denied any recent changes in diet/lifestyle and told me her recent medical checkup came out fine. On examining her rx profile I found out she was recently started on topiramate. I told her that topiramate is well known for causing appetite/weight loss and she may benefit from switching to a different anticonvulsant. She didn’t want to do that as her seizure was so well controlled on topiramate. Then I told her about other things she can do, such as eating small portions more frequently, spicing up her meals, and also going back to her doctor to get a presciption for an appetite stimulant. I also told her to talk to her doctor to rule out other causes of weight loss.
Now, Joseph Marconi, with your macaroni brain can you tell me how a computer could’ve done the same? Perhaps you can tell me if your computer tells you when and how to switch between different opioids. Does it also show you what to do if there IS an interaction between medications? Do you stop the meds? Do you lower the dose? Or is the interaction relevant at all?
Computers are great, but you would be a fool to think that a computer can replace your brain.

Evidently you are not aware of the mess the state of Florida is in re their new laws as a result of all the “pill mills”. It’s hard to get an Rx for any narcotic filled, no matter where you go. I have a brother who is dying from cancer [non Hodgkins T-cell lymphoma] and even HE cannot get his narcotics filled in Florida half the time and he LIVES there, has for the last 30+ years.

Why not just eliminate the FDA completely? I have been living abroad now for 5 years and managed to actually buy honest to goodness “prescription” (hushed reverance) drugs without spending $150 for a 1 minute drive by doctor visit. I have managed to avoid paying $450 for a single Amoxicillin pill.
It would be nice if you could, you know, talk to your doctor and then go get some medicine without praying before the baal of prescription compliance. If you stupidly take the wrong thing – you die – and that’s one less idiot in the world. Keeping morons alive is nice in theory, but many end up working for government agencies like the FDA and insurance companies and we now see how THAT worked out.

If they’re unsure which poison & which amount to use because they themselves are uneducated, well, then its up to that persons own judgement to say, “Hey, I don’know what I’m doing, maybe I should consult a physician or pharmacist”, but now days they may call it a consultation, however It’s not, It’s forced coercion, because w/o the so-called “consultation”, that person may die of pneumonia! Consultation implies its voluntary, but due to uncalled for government red tape, and inaccessible meds, its called forced coercion. I can no longer myself decide if I want to go to a doc or just go down to the drug store after self education. Self education? What a marvelous idea! Someone can actually properly educate oneself on the dangers and effects of meds? Who would’ve thought! And yes, I’m just as medically educated as some of u pharmacists & Dr’s, and yes I understand what things like Monoamine Oxidase Inhibitors do to the human body. They act by suppressingm a monoamine oxidase inhibitor transporterwithin the stomach allowing certain things to pass inside which generally wouldn’t such as so-called harmful schedule I drugs such asr such as me N, N-Dimethyltryptamine, wow, who woildve imagined a dumb ‘ole trucker such as me would’ve ever become self educated regarding organic and inorganic chemistry! Who wouldbevve ever thought wow! Anyways, theres no reason that a general public can’t make an informed decision even when it cones to narcotics & antibiotics. I’m a prime example… If a patient wants a narcotic bad enough they’re gonna find a way to get it whether legal or illegal. Marvel example me. My pain was being inadequately controlled. So just found me another doc that WOULD give me 100mg methadone per day & also a shrink who would supply me with 4 30mg adderall per day because the two a day previous doc gave wasn’t enough. The patient will get what they want regardless & there’s no point in making it illegal period. It’s rsr of all against our founding fathers principles as well as our constitution, which implementwd free will to men & supposed control over government intrusion, which we clearly see didn’t help one bit. Notice the drug wars a failure? Them being illegal anyway is a person violation of the constitution. Here’s a NOVEL IDEA, why don’t u guys fight for liberty and freedom for all, making it all legal, & how about making sure.u guys are there on standby for when an UNEDUCATED person comes along, instead of violating civil liberties & letting some bureaucrat decide our fate. Kiosks like these are a poor idea. Maybe the antibiotics, but i would limit that substantially, I know overuse of antibiotics have rendered some useless for some people. What I have once come across is a pharmacist who THOUGHT she was smarter than my doctor. I have been prescribed clonazepam, 2 mg in the morning, 2 mg in the afternoon and 1 mg at night (that is the current dosage it didn’t start that high), for over 10 years. Yes, generally, this is not for long term use. That sounds like a high dose, but is nowhere near the highest daily dose allowed. I have complicated migraine syndrome and severe anxiety. This is the only thing that works. At one point, I changed psychiatrists just over an office visit billing duspute. I was unhappy to change doctors after several years, but I needed a new one. Psychiatrists have long waiting lists. When I ran out of my medication, an ER physician gave me a 5 day script and referred me to a primary physician (my previous one had passed away). The primary physician gave me a 1 month Rx for substantially less per day when the one week script was up. It was a struggle, but she felt that since she wasn’t a psychiatrist, I should try to cope with less until I had an appointment with a psychiatrist. It took 2 months, and was difficult, but I did just that. I finally got in with a new psychiatrist and he gave me a script for 30 days at my original dosage. No prescriptions overlapped (it is a 30 day triplicate script, not possible where I live to overlap them). Same pharmacy. I go in with this script and a new hire pharmacist tells me she finds it odd ‘3 doctors in a short time’. I tell her the course of events, but she looks at me like I am a criminal. Her own computer would tell her I haven’t broken any laws or rules. NY state triplicate scripts are logged for her to see. She calls my dr, he is in with a patient. He is a one man operation. I tell her to say it is urgent, she never does. I go back to the pharmacy the next day, she is there again, no other pharmacist. She says she hasn’t heard back and won’t call again. I go the third day and the senior pharmacist is there, who been there the 7 years I have used this pharmacy. I’m in poor shape. I didn’t know I felt well enough to leave my house to go and check again. He apologizes. He had noticed the notes in her log and she had never actually followed up they way she should have. He looked further and the pharmacy indeed had the Dr’s emergency cell #. He informed me she was no longer employed there and waived my copay. NO, you don;t have to be police other than knowing state law and glancing at your computer screen.

I asked three differnt pharmacists for advice on how my 78 year old mother can remember to take her twice a day pills. One told me to put a psti=note on the medicine cabinet and the other two told me to call and reminder her twice a day. We live in the 21st century and there has to be something better. My nieghbor, who is a LPN told me about lifesavingpillbox.com. It is a electronic programmable pill dispeser that makes mistakes impossibel. Cost less than 60 dollars (1/2 the price of on script) and she hasn’t missed a pill in eight months.
I wish I has been told about this sooner, as it would have prevented tow ER trips.

Anyway can someone tell me where to find the information on why my local drugstore was closed by the FDA or the Feds.?
Is there a website that informs customers why their drugstore has been closed down?

It’s not playing god, it’s following the law. Which if you don’t do, will result in losing your license.

hy mr farmasitant any hot dog stand can refuse service they don’t have a degree its like he wanted to sell hamburgers like you wanted to be a doctor he don’t sell hamburgs and you aint no doctor
